Provide the full name and title for the head of the research group requesting academic licenses (e.g. Prof./Dr. Douglas Oldenburg). Note that students and non-academic employees will NOT be granted licenses.

Provide a title for your current position at the aforementioned academic institution. Once again, the applicant must be a professor or the head of a research group.
Email addressed must be attached to an accredited institution. Submissions using Gmail or other third party email addressed will NOT be accepted.

It is our intent for academic license codes to be used for research that will be published in public spaces. Please specify where and how you plan to publish research made possible through the use of these codes.
The codes you license will operate ONLY on ONE computer running the MS Windows operating system. You are REQUIRED to supply the physical address number associated with the network card on the computer that will run the inversion code. This is entered below.

The physical address number is defined by 10 alphanumeric characters separated by hyphens and has the format
"XX-XX-XX-XX-XX-XX". To find the physical address number for the computer:
  - Open a command prompt on the intended computer
  - Type 'ipconfig /all'
  - Look for the physical address number (preferrably for an ethernet connection)
